Check In Important Info

• Valid Player Cards
• Approved Roster from your club(ones you use during league)
• Medical Releases
• Birth Certificates(in case any controversy)
• Travel Papers if traveling from out of state

We also need all teams to fill out the below form and print it on a heavy card stock paper and bring to check in. 

You will use this card for all of your matches and collect from the referee after the game.
